Founded in 1992, Sonoma Valley Community Health Center (SVCHC) is a non-profit 501c3 Primary Care Community Health Center located in Sonoma, CA. Designated as a Federally Qualified Health Center (FQHC) 330 clinic in 2006, we are located in the isolated southeastern corner of Sonoma County, roughly 50 miles northeast of San Francisco.
Our service area, with an estimated population of 48,000, includes the city of Sonoma and adjacent communities, and a vast agricultural region which supports the area’s winemakers. Three full-time licensed facilities are operated by SVCHC, including a mobile medical van which serves our surrounding communities. In 2011 alone, we provided care to over 7,000 unique patients, comprising over 27,000 visits.
Our mission is to provide accessible quality healthcare to those who need it, especially the underserved. We serve our community through comprehensive primary care, pediatrics, well-child exams, immunizations, urgent care, family planning, community health outreach, and nutritional counseling.
