We are proud of our reputation among FQHC's as being at the forefront of community health, we offer comprehensive primary care services to a diverse population, including some of the most vulnerable in our county.
Our team-based model of care offers clinicians an opportunity to invest deeply in the lives of patients and families while working collaboratively with a highly effective team to support patients through the lens of whole-person care. Teams include RN Care Managers, Behavioral Health Providers, In-house Psychiatry, Care Navigators, legal services, educators, integrative health practitioners and specialized staff for HIV, Hepatitis C, Dental, Obstetrics, Addiction, Homelessness, Gender Expansion, Teen Care, Day-labor, and Telehealth.
West County Health Centers offers unique services including our internal Innovation Department that uses ‘human-centered design, to help staff and community partners approach challenges as opportunities and create novel and creative solutions. Our Quality Team uses big data analytics, data visualization and geo-mapping to offer strategic insights to front-line staff and leadership for population management, public health initiatives, disaster response, and community-based projects.
