CONNECTING COMMUNITIES THROUGH HEALTH

UMMA Community Clinic has been the medical home to over 25,000 adults and children living in South Los Angeles. For our patients, UMMA is possibly the only source of health care, and without us, many conditions would go undiagnosed or untreated.

UMMA operates two facilities in the South Los Angeles area, our flagship clinic on Florence Ave, celebrating its 20th year of operations in 2016, and our school-based health center the Fremont Wellness Center on the campus of the Los Angeles Unified School District’s John C. Fremont High School.

UMMA emphasizes the commitment to quality and safety for our patients by providing care based on evidence-based clinical guidelines; demonstrated by our Program Level 2 certification as a Patient Centered Medical Home from the National Committee for Quality Assurance. Our primary care providers are supported through a care team-based approach that includes medical assistants, a licensed vocational nurse/case manager, referral coordinators, a licensed clinical social worker, health educators, and a contracted pharmacy. We are an electronic medical records facility that utilizes eClinicalWorks. UMMA is a Medicaid and Medicare provider and a member of the Health Care LA IPA. Our patients reflect the community we serve: 66% Hispanic and 25% African American, 51% of patients are best served in a language other than English, and 74%, are at 100% or below the Federal Poverty Level.

UMMA envisions itself as part of a larger network of institutions addressing the health and well-being of the underserved and indigent, mindful of the cultural, spiritual, social and economic realities that impinge upon them and the traditional barriers to accessing care.

Paul Getty Museum, the Los Angeles County Museum of Art, the La Brea Tar Pits (famous for Ice Age fossils), Disneyland (Anaheim), and the Santa Anita and Hollywood racetracks.
