Operating uninterrupted for over 100 years, UNION has emerged as one of the largest Federally Qualified Health Centers (FQHC) in New York State. Located in one of the most culturally diverse boroughs of New York City, UNION has grown to six locations conducting nearly 200,000 patient care visits annually to an active patient caseload of over 37,000 individuals. In 2016, UNION ranked in the 97th percentile by HRSA for clinical, financial, and regulatory performance. As a teaching facility, UNION trains the next generation of health care professionals to understand the complexity of serving low-income, uninsured, disadvantaged and indigent populations in a cost-effective manner. UNION is training healthcare professionals to meet patient care challenges in a value-based payment environment.

Union Community Health Center provides comprehensive primary medical care to pediatric, adolescent, adult, and geriatric patients.

Primary medical care is provided by Board-certified family physicians, internists, and pediatricians.

Union Community Health Center is recognized by the National Committee for Quality Assurance (NCQA) for its medical home model of providing comprehensive, quality care.

Bronx, NY

The Bronx is the northernmost of the five boroughs of New York City. Coextensive with Bronx County, it was the last of the 62 counties of New York State to be incorporated. Located north of Manhattan and Queens, and south of Westchester County, the Bronx is the only borough that is located primarily on the mainland.

About one-fifth of the Bronx's area and one-quarter of its land area, is given over to park land: about 7,000 acres. Nearer the borough's center, and along the Bronx River, is Bronx Park. Its northern end houses the New York Botanical Gardens, which preserve the last patch of the original hemlock forest that once covered the entire county, and its southern end the Bronx Zoo, the largest urban zoological gardens in the U.S.

Education in the Bronx is provided by a large number of public and private institutions, many of which draw students who live beyond the Bronx. The New York City Department of Education manages public noncharterschools in the borough.
