Tri-State Community Health Center was incorporated as a private non-profit entity in 1987. Tri-State was established by the community to help assure that comprehensive primary healthcare services would be available to the area’s underserved, rural population with a high concentration of low-income residents. Tri-State operates five community health center sites that offer community-based primary care, preventive health services, and ob/gyn.

The sites are located in Allegany and Washington counties in Maryland; Fulton County in Pennsylvania; and Morgan County in West Virginia. These offices serve residents in each of the respective counties, as well as the surrounding areas. Although the primary target population for Tri-State Community Health Center is low-income, uninsured and underinsured individuals and families, we serve all patients with and without insurance.

The organization has grown steadily in health center users and medical visits due to the expansions and the increase in need. In 2013, Tri-State provided services to 17,165 community residents facilitating almost 60,000 encounters. These services are currently provided through the employment of physicians, nurse practitioners, physician assistants and a wide range of clinical and administrative support staff, creating over 120 jobs in the communities served by Tri-State.

Tri-State offers, directly and through collaborative arrangements, a wide range of general, primary care and health maintenance services, CLIA-waived diagnostic laboratory services and other diagnostic tests and screenings, family planning, urgent medical care, after-hours coverage, immunizations, HIV testing, gynecological care, mental health treatment and counseling, developmental screening, vision screening, hearing screening, oral health screening, health education, transportation, translation and case management services. Services provided off site through referral arrangements include obstetrical care, mental health and substance abuse treatment and counseling, developmental screening, oral health services, crisis intervention and hospital care.

McConnellsburg's historic district was recognized by the United States Department of the Interior in 1993 when it was listed in the National Register of Historic Places. The district consists of 144 structures that contribute to its historic character. Of notable meaning are the numerous taverns, inns, automotive garages and other travel-related structures still in existence today, which includes Daniel McConnell's original log home.
