Opportunity for professional collaboration with our 50+ clinicians encompassing family practice, pediatrics, internal medicine, ob/gyn, nurse midwifery, psychiatry, psychotherapy, dentistry, pharmacy. On-site Lab, Radiology, Pharmacy.

Community Health Center with Joint Commission Medical Home status. Coordinated services include Integrated Medical/Behavioral Health, Drug-Free Moms, and Babies program, Diabetes Collaborative, HIV/Aids program, Migrant Health program, Nursing outreach and referral for Homeless population, WIC, Social Work support, Health Education and Support groups, RN Postpartum Home Visits, Patient Transportation.
