Seward Community Health Center is a federally-qualified health center and recognized Patient-Centered Medical Home. We offer high-quality medical care for a variety of illnesses and conditions – from the very young to the very old.  We take pride in removing barriers to care through services such as transportation assistance, prescription assistance, translation services, sliding fee discount program, and more.

Our team-based approach to care puts patients first and recognizes there are many elements that go into making a person whole and healthy. Behavioral health is integrated with primary care here to provide our patients with immediate access to mental health and substance abuse treatment.

It takes more than doctors to make an effective clinical care team. Board-certified providers are but one part of a provider team that includes mid-level providers, registered nurses, behavioral health professionals, medical assistants, and administrative staff. Our dedicated and knowledgeable staff work together to create a caring work environment to benefit our community.
