Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers through clinical programs and education. We provide primary care services (medical, dental and behavioral healthcare) through 27 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert and Kit Carson counties. Our organization has over 950 employees and serves over 95,000 patients in the Pikes Peak and East Central Plains region.
Peak Vista Community Health Centers has partnered with the local El Paso County Department of Human Services (DHS) to create the Home Visit Program. In this program the medical provider partners with a DHS caseworker to conduct in Home Visits providing services to and assessments of suspected Child Abuse and Neglect focusing on the 0-3 year old population; evaluation and the treatment of Acute illnesses where appropriate and working closely with DHS to assist patients with accessing a Patient-Centered Medical Home.
