MVHC is recognized by NCQA as a Level 3 Patient-Centered Medical Home. As a PCMH, we have developed a model of care that supports not only the Patient but the Provider, as well. Our team consists of an RN Care Coordinator and two clinical support staff. MVHC also has integrated behavioral health, which is of great assistance to the care team in identifying and removing socioeconomic barriers to optimum health.
