FoundCare is a not-for-profit Federally Qualified Health Center located in Palm Beach County, Florida.
In order to accomplish its mission of fulfilling unmet healthcare and social service needs of individuals and families in Palm Beach County, FoundCare operates a Federally Qualified Health Center that offers pediatric and adult primary care, chronic disease management, behavioral health services, laboratory and X-ray services, dentistry and pharmacy services. FoundCare has two health centers in Palm Beach County: one in the Village of Palm Springs and the other in North Palm Beach. FoundCare accepts most private insurance, Medicaid, and Medicare, as well as a sliding fee discount scale based on household size and income for individuals without insurance. In 2016, FoundCare served 14,807 patients for a total of 51,992 visits. FoundCare is a National Health Services Corps (NHSC) loan repayment site with a primary care HPSA score of 16, a dental HPSA score of 23 and a mental health HPSA score of 21.
