- The physician will be joining 1 MD, 1 FNP, and 1 LCSW

- Telehealth and remote patient monitoring projects for HTN/DM underway
- The patient population is mostly adults.
- We are an NCQA-Recognized Patient-Centered Medical Home Level III.

QUALIFICATIONS:
- 2 years of experience preferred.
