Florida Community Health Centers, Inc. (FCHC) is a private, non-profit, Federally Qualified Community Health Center with multiple locations. Our mission is to provide comprehensive, cost effective primary and preventive care to all patients regardless of their socioeconomic status. Our focus is on chronic disease management and preventive care/screenings, inclusive of Women’s Health Services. Laboratory services, Care Management, and Social Services enrollment assistance are available on site. Centralized Referral assistance is available to our patients.
We are Joint Commission accredited and all of our primary care sites are recognized by NCQA as level 3 Patient Centered Medical Homes. We use the NextGen EHR system. All new staff receive both group and individualized EHR training via an assigned Trainer during orientation week.
Clinician training is extended appropriately. Additionally, each practice site has a trainer available full time to provide updates as needed. Our sites are NHSC loan repayment locations and have competitive benefits, including a pension plan, 401K plan, competitive insurance options, free dental insurance, CME days and allowance, and professional memberships.
Our patients and staff are culturally diverse. We have experienced a significant amount of growth in the past few years, both in number of patients and number of sites. We have a strong focus on quality, and are continuously working to maintain our status as a leader in improving our quality indicators, such as HEDIS measures. Further information on our QI and other programs is given during our extensive orientation week. The languages most commonly spoken by our patients, after English, are Spanish and (French)Creole.
