Our mission is to provide comprehensive, cost effective primary and preventive care to all patients regardless of their socioeconomic status. Our focus is on chronic disease management and preventive care/screenings, inclusive of Women’s Health Services. Laboratory services, Care Management, and Social Services enrollment assistance are available on site. Centralized Referral assistance is available to our patients.
