Florida Community Health Centers, Inc. (FCHC) is a private, non-profit, Federally Qualified Community Health Center with multiple locations. Our mission is to provide comprehensive, cost-effective primary and preventive care to all patients regardless of their socioeconomic status. Our focus is on chronic disease management and preventive care/screenings, inclusive of Women’s Health Services. Laboratory services, Care Management, and Social Services enrollment assistance are available on site. Centralized Referral assistance is available to our patients.
We are Joint Commission accredited and all of our primary care sites are recognized by NCQA as level 3 Patient-Centered Medical Homes. We use the Athena EHR system. All new staff receives both group and individualized EHR training via an assigned trainer during orientation week.
Clinician training is extended appropriately. Additionally, each practice site has a trainer available full time to provide updates as needed. Our sites are NHSC loan repayment locations and have competitive benefits, including a pension plan, 401K plan, competitive health insurance, free dental insurance, free long-term disability insurance, free life insurance, paid CME days, a CME allowance, 18 vacation days, and professional memberships.
Our patients and staff are culturally diverse. We have experienced a significant amount of growth in the past few years, both in the number of patients and number of sites. We have a strong focus on quality and are continuously working to maintain our status as a leader in improving our quality indicators, such as HEDIS measures. Further information on our QI and other programs is given during our extensive orientation. The languages most commonly spoken by our patients, after English, are Spanish and (French)Creole.

Seven miles of majestic towering Royal Palm trees greet visitors driving into Pahokee. The tropical climate, moderating balmy breeze from Lake Okeechobee and nature's richest organic soil, called "muck" makes our town a natural home for the Royal Palms.
While the rest of Florida has boomed, Pahokee has maintained its balance and its sanity. These days, we're finding more and more folks come out just for that reason. While this website is dedicated to giving our visitors an introduction to our town, its also dedicated to understanding the pride each of us feels for our town.
