The Erie County Community Health Center is a Federally Qualified Health Center (FQHC) recognized by NCQA as a Patient Centered Medical Home and located on the shores of Lake Erie in Sandusky, Ohio.
The mission of the Erie County Community Health Center is “To provide affordable, quality, comprehensive primary and preventative health care across all life cycles in a patient-centered medical home environment. The Erie County Community Health Center is a modern facility, located on Sandusky Bay, with state of the art equipment for medical and dental visits.
Located on Sandusky Bay, Sandusky has over 22 miles of shoreline within the city limits. We are the governmental seat of Erie County and are noted for our beautifully landscaped parks, our historical architecture, and the Sandusky Bay Pathway.
The Sandusky area is usually solely identified with Cedar Point, the park that has been honored by Amusement Today to be the best amusement park in the world, but this area also has shopping, museums, boating, cruises, water parks, golfing, fishing, camping, spelunking, dining, wineries and much more.
The Sandusky area offers so many outdoor activities from bird watching, gardens, bicycle trails, golf courses, state nature preserves, lighthouses, and even a few caverns.
