- Damian Family Health Center is a Federally Qualified Health Center.
- Facilitates, plans, and organizes the provision of Social Work and Psychotherapy services for patients.
- 3 years experience post MSW.
- Current LCSW NYS license and registration.
- Minimum of one-year recent experience in an ambulatory care setting required.
- Experience with chemical dependence, Medication Assisted Treatment (MAT), and HIV services preferred.
- Must possess excellent organizational and communication skills.
- Bi-lingual skills in English and Spanish are preferred.
- Must be able to collaborate well with others.
- Experience documenting in an EMR.
