Job Description

Hours: Monday - Friday 7:30 AM - 6 PM | 5 Day Work Week
Employed
Average patients seen: 18-21 | 2.5 per Hour
Call Schedule: Phone Only | 1:3 | Rotated Weekly

Compensation:
- Starting at $90K Based on Experience

Additional Information:
- Minimum of 2 years of experience with chronic disease management
- DEA and suboxone certification or willingness to obtain a huge plus but not required.
- Patient mix 35% pediatric, 45% adult and 20% geriatric.
- Our clinics use an integrated behavioral health approach.
