Job Description

Hours: Current schedule includes four 9 hour shifts Mon-Thurs followed by a half day on friday to make a full time 40 hr position. Locations are subject to change Mirrors Primary care hours. 8:00am to 5:00pm
Average patients seen: Less than 10
Call Schedule: No Call

Additional Information: Portland, OR - Providence Medical Group is seeking an excellent Primary Care nurse practitioner or physician assistant to join our team supporting several clinics in the Portland Metro area, focusing on Annual Wellness Visits for Medicare patients only. Experience all that the Pacific Northwest has to offer from the beautiful and welcoming city of Portland.
* Full-time position. At least 2 years of post-graduate, Primary Care experience required.
* Experience working with older, geriatric population preferred.
* Provide continuity of care by developing and implementing patient management plans.
* Reviews preventative wellness screening information and engages the patient in creating a plan for optimal wellness
