Hours: Flexible schedule and remote work opportunities
Employed
Average patients seen: 40-50 Attempted Outreaches
Call Schedule: No Call Other Than Weekend Work as Needed for COPD Alert Coverage

Additional Information:
- The physician assistant can live outside of Louisiana but must have a Louisiana license.
- At least 2 years' experience required, ideally in primary care

- Tremendous opportunity to be part of the exciting digital transformation happening in medicine!
- Work as part of a multi-disciplinary team to provide remote medication management services to patients in digital medicine programs for chronic disease management (hypertension, diabetes, COPD, etc.).
- Leverage technology to optimize patient care in a novel delivery model
